Accident investigations normally take two reactive steps:
Step 1: The accurate identification of the root cause and contributory factors of any accident is key to the whole process. Under the Safety, Health and Welfare at Work Act 2005, employers have a general duty to report prescribed accidents and dangerous occurences to the Health and Safety Authority.
